Some time--a gift of your time.
To be taken out to dinner by you, not for her birthday, but on a Tuesday, when nothing special is going on.
A book of stamps, a box of assorted greeting cards, as it is hard to get to the store and she would like to send birthday, sympathy, etc. cards.
Pre-cooked, home-made meals, individually packaged, so that she can freeze them, then take them out and enjoy them when she wants to.
A home-cooked meal, brought by from time-to-time, and you to eat it with her. It's just food if it is dropped off; it's a meal when enjoyed with a friend!
Gift certificates for her heating, gas, electric bills.
Gift certificates for the grocery store, local restaurants that deliver, gift certificates for the local chain drug store, Wal-Mart, etc.
An offer to drive her to the doctor, dentist, etc.
An offer to clean her home, or a maid-service gift certificate.
A chance to spend some time with you, and a lingering hug when you leave. Older folks really, really miss being touched and hugged. Often, their mates have died, their children and grand-children are grown and too busy, and they are touch-deprived. It is a sad but true fact of getting older.

if she is like I am, I don't want things that I don't need, won't use and when I die someone has to get rid of the gifts
do something nice for her, maybe treat her to a special meal , or just give her lovely flowers
